General Price List

Full Service Care: At Evergreen we have all the facilities necessary not only to care for your loved one, but you as well. We have a chapel and reception center on-site for use during services. Prices do not include cost for: Casket (if applicable), Sales Tax, Death Certificate, Ancillary Items such as flowers, memorial books, and folders, etc.

Traditional Services

Traditional Church/Chapel Services: 3,250.00

Includes minimum service of Funeral Director and Staff, transfer of the deceased to the funeral home, embalming, dressing, casketing and cosmetizing, staff and /or equipment for a church/chapel service, funeral coach, and service vehicle.


Graveside Service:  3,105.00

Includes minimum service of Funeral Director and Staff, transfer of the deceased to the funeral home, embalming, dressing, casketing and cosmetizing, staff and/or equipment for a graveside service, funeral coach, and service vehicle.


Direct Burial 2,215.00

Services include minimum service of Funeral Director and Staff, transfer of remains to funeral home, refrigeration, dressing and casketing, unscheduled delivery of the casket, and service vehicle.

Cremation Services

Traditional Chapel Service with Cremation:  4,600.00

Includes basic services of Funeral Director and Staff, transfer of the deceased to the funeral home, embalming, dressing, casketing and cosmetizing, staff and/or equipment for service, funeral coach, service vehicle, rental casket, casket insert, temporary urn, and crematory fee.


Memorial Service with Cremation: 2,970.00

Includes basic services of Funeral Director and Staff, transfer of the deceased to the funeral home, refrigeration, staff and/or equipment for Church/Chapel memorial service, service vehicle, crematory fee, cardboard cremation container, and black plastic urn.


Direct Cremation: 2,150.00

Service includes minimum service of Funeral Director and Staff, transfer of deceased to the funeral home, refrigeration, service vehicle, crematory fee, cardboard cremation container and black plastic urn.

Receiving Remains From Another Facility: 1,355.00

Service include minimum service of funeral director and staff, transfer vehicle from airport (if necessary), and transport to funeral home. ( This price does not include any charges for staff or facilities for services or visitation. Additional service charges will be added upon the selection of services to be rendered.)


Forwarding Remains to Another Facility: 3,308.00

Services include minimum service of Funeral Director and Staff, transfer of remains to funeral home, embalming, service vehicle, transport to airport and a standard combination unit. (Airline or other shipping charges are not included in this price.)


Minimum Services for Funeral Director and Staff: 1,215.00

This charge includes consultation with the family or responsible party; staff and facilities to respond to initial request for services. Coordination services plans with the cemetery, crematory, and other parties involved; securing official documents and necessary permits, and liability of sheltering remains. This charge for our basic services and overhead will be added to the total cost of the funeral arrangements you select. Such a charge is already included in our charges for direct cremation, immediate burial, and the forward or receiving of remains.

Other Preparations

  • Embalming: 425.00 Except in certain special cases, embalming is not required by law. Embalming may be necessary if you select certain funeral arrangements with visitation.

Services Unique to Cremation:  When there is not a request for embalming, refrigeration is required by state law 24 hours after death (CRS 12-54-111) until cremation can take place. So that the deceased remains are in a refrigerated state, we respectfully request that viewing for family be no longer than 20 minutes from the time we agree to begin. There is no facility or staff charge for the 20-minute viewing period. There is a fee of 125.00 for preparation of the deceased.


Direction of Services & Use of Facilities

  • 595.00:  Services of Funeral Director and Staff, use of equipment in organizing, supervising and direction of funeral services. Includes coordination with those providing other portions of the ceremony such as the cemetery, minister, music, fraternal organizations, military personnel, pallbearers, attendants to floral and other tributes to be arranged before services begin. Our professional and compassionate staff act as a host to family and friends attending the service, guiding and supporting bereaved family, ushering guests, supervising parking, ensure guests sign a memorial book and receive a memorial folder, take memorial contributions, and all printing of memorial accessories.
  • 450.00: Services of Funeral Director and Staff, use of equipment in supervising and directing of graveside services. Includes the same services list above.
  • 250.00:  Use of facility and services of Funeral Director and Staff in supervising visitation
  • 359.00: Use of facility for gathering
  • 500.00: Additional Charge  for Saturday
  • 800.00:  Additional Charge for Sunday

Insurance Payments

Insurance policies may be accepted in lieu of payment for all services except for Cash Advance Items, which MUST be paid before services are provided in one of the accepted payment types. All insurance policies MUST be confirmed as valid and assignable through the Insurance Carrier to the Funeral Home. Once confirmed to be valid, an insurance assignment form must be signed by the beneficiary, notarized and the original policy to the Funeral Home.

Department of Human Resources (Medicaid Cremation Services)

Due to Department of Human Services (DHS) strict spending limits on Medicaid recipients, the funeral home will only be able to assist families choosing to use Medicaid death benefits for cremation and memorial services. Only upon payment in full from the family and/or written verification from DHS of payment forthcoming, may a memorial service or cremation be scheduled with our company. Please note that verifications of payment from DHS can take several weeks and sometimes even longer if the family fails to fully cooperate with DHS. To qualify for DHS death benefits, your family member must be receiving Medicaid benefits prior to death. Medicaid funded memorial services cannot exceed 2,500.00. In most cases for people 60 years of age and older, Medicaid will pay up to 1,500.00. People under 60 years of age can receive up to 1,000.00 (infants and children- up to 1,500.00). The caseworker will examine the assets of the deceased before determining the Medicaid benefit. The existing assets are then deducted from any death benefit would be paid. For example, if a 67-year old woman passes away and had 600.00 in a savings account, Medicaid will only pay 900.00 leaving a 600.00 balance due. The person who signs the funeral home contract is responsible for the difference between the benefit from Medicaid and the bill. The balance must be paid in full at the time of the arrangements and prior to any services being rendered.
